32H7 Board Not Connecting - "No STM32 Target Found" Error

I am facing an issue where my STM32H7 board is not detected using STM32CubeProgrammer. I am using an STLINK-V3SET programmer, and the following error keeps appearing:

Error log:

Setup Details

1. Board: STM32H745xi series
2. Debugger: STLINK-V3SET
3. Software: STM32CubeProgrammer 
4. Voltage: 3.3V confirmed

Please give details; also versions, etc: How to write your question to maximize your chances to find a solution

Has it ever worked or is this a brand new board/design? Are SWD/NRST pins connected? Show schematic, details.
