I run into this problem every time I debug. No matter what I did I couldn't fix it. Even though I selected Debug configuration>Debugger>ST-LINK(OPENOCD) and Reset mode>software system reset it still works. I made a chip earese from the ST-LINK UTILITY program, but it didn't work.

Hello there!
Is it the same issue if you try ST-Link GDB server, for example?
Same issue for many boards/projects or just this one? If you're not sure, could you try with another board and/or example?

Hello, are you sure you are disconnected from other tools like STLink Utility before trying to debug ?
If still not working, try to update CubeIde since Openocd version is dated from 2019.
