can the STM32H745BIT6 operate continuously at temperature limit?

The "operating temperature range" is specified as +85 deg. C high limit.

Does this really mean the MCU can operate with an ambient temperature of 85 C ?

Only the maximum junction temperature is specific in the data sheet.

You can estimate the internal ("junction") temperature by adding to ambient the temperature gradient given by thermal resistance (given in DS for each package, this assumes the chip is soldered on a decent area of PCB with copper, and there are no other sources of heat on that PCB) * power consumption (supply current * voltage).
