Theres T1 T2 T3 T4 and QA. QA is unchecked. Assuming it means Quality Assurance. Should I be worried?

There is no need to worry, as this situation is normal.
Practically, the Tx box is checked when the corresponding assembly phase is successfully completed. The number of phases varies by product reference. For example, T1 indicates that the product's SMT assembly was successfully completed, while T4 signifies that the final test and demo flashing were passed.
The QA box, when checked, indicates that the product has undergone a comprehensive quality inspection as part of random sampling. This check is thus optional.
In your case, the product did not undergo this random quality inspection.
