STM32WB5MMG module. Can this be used in products with varying application (CPU1) code without cert of BLE or RF? Any real-world answers such as it cost us $ dollars and X months would be welcome.

For both areas of certification, the customer should anyhow contract a test house with relevant expertise.

For BLE, customer needs to go through the product declaration process defined by SIG.

The RF-PHY and hosts stacks have been certified, so the overall testing and cost will be less than if the development was started from scratch.

For RF, the design of the end product may have some impact on the RF performance.

To get the certification needed for FCC and CE labels, it is likely the test house will want to double check at least any spurious emissions.

Is it possible to gain access to the lowest layers of the radio core such that you can write code that does not use Bluetooth and hence would not require certification as a bluetooth product?

These materials tend to be closely held. I'd suggest chatting with your local ST sales rep to understand what documentation, code and volumes would be needed.
