Is it mandatory to use an external DC/DC converter with STM32L496 with external SMPS package? Or would it just affect the consumption in run modes?

The external SMPS is intended exclusively for the possibility of maximum energy saving if you supply the STM32 e.g. via a battery or from an energy harvester.
Of course, you can use the STM32L496 without an external SMPS and only with an internal LDO, but then you will burn up considerably more electrical energy, as you can see from the corresponding parameters in the data sheet.
