MASTERGAN1TR failing and smoking at 100VDC

We are using 2 MASTERGAN1TR to drive current across an inductor.
We are seeing damage on the body of the chip nearest the low side ground connection with a PWM Frequency of 250KHz and a 12% duty cycle.
Before the chip fully fails the current decreases on the VS rail and the VCC/PVCC, which was previously stable at 91mA, climbs to 350 mA and the chip blows.

Operating Conditions:
VS - 100V w/ 1.5Vppk ripple on switch and .2V Decay during active period
PVCC to PGND -  5V w/ .1Vppk ripple on switch and 50mV decay during active period
OUTb to BOOT - 5.1V w/ .3V sag on switch

These measurements all fall within operating conditions laid out in the datasheet.
Since the each Half Bridge is only switching the high side or only switching the low side, shoot through is impossible.
