Why are some notifications missing icons?

Is this intended?

A complex system that works is invariably found to have evolved from a simple system that worked.
A complex system designed from scratch never works and cannot be patched up to make it work.

I don't remember seeing anything like this. I'll start to pay more attention now on.

The strange thing in this is, that the given icon *is* already loaded, as there are other "replied" links at the same page, so this can't(?) be the case of a file fetch error.

@Andrew Neil, what browser is this?

It was Chrome on Win11-Pro.
