Hello,
I am trying to understand the datasheet regarding current draw of the STM32WB55 during BLE transmission. I see TX maximum output power consumption (SMPS Bypass) 12.7mA but what I don't understand is... does this include the Cortex M4 processing along with the M0?
Could I assume in this configuration a program transmitting a BLE stream (running at 64MHz) I would read ~12.7mA (MAX) from the power supply?

12.7 mA is indeed the complete power consumption level of the STM32WB (both M4 and M0+ and all peripherals).
Note that it is an average. You may see higher peaks of current for a short time when an RF activity is on-going.
