Issue while connecting to STM32H7A3 device over ST-Link

I am using STM32H7A3ZIT6Q board. While debugging I am facing an issue like "No ST-Link server detected" and also sometimes it is giving error as "Error in Lanching, Failed to execute MI command". How to solve this issue and why we are facing this issue?

Ensure your power settings are correct. The board is set up to use SMPS so the power setting in CubeMX on the RCC tab should be:

It's also possible you have a bad USB cable or port, or you have firmware loaded which doesn't allow for easy debugging such as erasing flash or entering low power states.
