How should I configure DCMI->ESCR and ESUR registers for embedded synchronization?

I read about SAV EAV codes, but there should be 8 variation (line start, line end with and without VB for each Filed (0|1)).

So there are only 4 position, how should I configure this registers for correct work with ITU-R BT.656?
