Do I need flyback diodes at the output for bipolar steppers with the STSPIN220 chip? I've been looking at examples and everything show direct connections. Shouldn't I be worried about ESD and transients? Thanks for your help.

The internal MOSFETs already have freewheeling diodes, their forward voltages have been specified in the datasheet with typ. 900mV at 1.3A, i.e. at the max. driving current.
Whether you should use Schottky diodes in parallel with the internal freewheeling diodes depends on your EMI, efficiency, and total cost needs.
