No. As stated in the post, it is STARTUP FAILED. 

 I have read App note AN1276 and the issue does appears to be in the alignment ramp and startup ramp table. How can I compute the correct parameter/table values for these functions? What other parameters must I set to get the system working?

Is the fault condition Overcurrent?

I received a spreadsheet from ST online support that computes a startup ramp based on PWM freq, pole pairs, and rotational speed (rpm/s). I was able to install the new table in the code and now the motor runs fine.
